Fees and Qualifying for Services:

NOW ACCEPTING MEDICARE AND MEDICAID

For patients who do not have medical coverage, visit fees will be based on family size and household income, according to a sliding fee scale.  The minimum charge is $20.00 and maximum charge is $135.00 per visit.   Upon checkout the client will be responsible for paying fees based on the services performed during the  medical exam (i.e. procedures, labs, testing, etc.) based on the sliding fee scale.

Please bring the following information with you to your appointment to make the registration process go faster:

Proof of income (required to qualify for discounted sliding fee scale).

bullet

Examples include: pay stubs, tax returns, or a letter from your employer.

bullet

Copy of insurance card (Medicare, Medicaid, private insurance).

bullet

Immunization record (if applicable).

bullet

All medications you are currently taking (prescription, herbal, and/or over the counter).

bullet

Address verification (utility bill, phone bill, etc.)
