For patients who do not have dental coverage, visit fees will be based on family size and household income, according to a sliding fee scale. Upon check-in the client will be responsible for paying fees based on the scheduled services to be preformed during the dental appointment (i.e. procedures, exam, x-rays, etc.) based on the sliding fee scale.
Please bring the following information with you to your appointment to make the registration process go faster:
Proof of income (required to qualify for discounted sliding fee scale). Examples are pay stub, tax returns, or a letter from employer.
Copy of insurance card (Medicare, Medicaid, private insurance)
All medications you are taking (prescription, herbal, over the counter)
